Here is a sample letter you can to give your bishop or clerk to help them understand how you can participate in the out-of-unit membership record program. Change the information in the letter to fit your needs.

Dear Bishop and Ward Clerk,
The Utah Area Presidency has assigned the Utah Valley Ward (Sign Language) to be an agent ward for deaf or hard of hearing members who do not live near an dedicated sign language unit in North America. As an option for deaf or hard of hearing members and following the guidelines found in the General Handbook, 33.6.5 (‘Out of Unit Member Records’) and 33.6.11 (‘Records of Members Who Are Deaf or Hard of Hearing’), this will allow me to participate more fully in the gospel community and in weekly church services in a dedicated unit that uses American Sign Language.
I understand that I will need my local bishop’s authorization for
receiving the sacrament (General Handbook, 18.9.1)
receiving or renewing a temple recommend (General Handbook, 26.3.2)
help with self reliance and welfare needs (General Handbook, 22.5.1, 31.3.4)
The ward clerk can create an out-of-unit membership record and transfer it to the Utah Valley Ward (Sign Language) by following the steps at https://www.churchofjesuschrist.org/tools/help/out-of-unit-members-and-callings?lang=eng. The Utah Valley Ward clerk and assistant ward clerk responsible for this program can be contacted at uvwardclerk@gmail.com.
